Fees for non-NHS Services 1^st September 2026

This list is for guidance only, not all forms and certificates and listed. Please ask if you have a different form for completion (The fee will reflect rates below based on the time taken to review the records and complete the form).

The NHS does not pay for all of the services that we provide. Our fees for these services are listed below. There will be occasions when medical forms require specialist knowledge which is beyond the expertise of our GPs and in these situations, we will advise you seek an alternative provider to complete the form. This includes sports medicals and most ‘fitness to participate’ forms.

Some items and services are subject to VAT and this is applied at the standard rate.

When a doctor signs a certificate or completes a report they can only sign what they know to be true. In order to complete even a simple form, the doctor may need to check the entire medical record. Carelessness or an inaccurate report can have serious consequences for the doctor with the General Medical Council or even the Police.

The new VAT rate (20%) takes effect from 25th May 2018.
